• Opening the conversation & encouraging a person centred
plan
• Advance care plans & advance statements
• LPA & Advance decision to refuse treatment
• Sharing wishes & preferences
Learning Outcomes
At the
end of the session colleagues should be able to:
Understand the importance of Advance Care Planning
Identify what might be important to a patient
Demonstrate how to open a conversation about ACP
Differentiate between an advance care plan & an advance statements
Define LPA & ADTRT
Be
able to facilitate advanced care planning appropriately and sensitively
Introduction
to palliative care principles and how to carry out a holistic needs assessment.
