Course Information
In this module we look at how we can provide support & care for patients who are in their last year of life, which can enable them to remain at home. This will include a session from the Liverpool STARS programme about the importance of providing physical care to patients. A social worker will also discuss their role in providing psychological support to patients and their carers’ in the community.. We will also hear about the wide range of other services that a hospice can offer including day therapy.
Learning Outcomes
At the
end of the session colleagues should be able to:
- Identify support services that can be accessed for a patient in the community
- Describe the importance of providing psychological & spiritual care to patients & carers
- List the services that a hospice can provide.
- Be
able to facilitate care planning appropriately
