- Gain an understanding of the techniques used within a Root Cause Analysis approach.
- Understand how to use the Incident Decision Tree as developed by the National Patient Safety Agency.
- Understand what a systems approach is.
- Be able to use tools such as Barrier Analysis, Time / Person grid and fishbone Diagram during the review process.
- Have increased confidence in investigating serious events.
- The course will walk you through a RCA on Day 1 and then on Day 2 give you an opportunity to practice your newly learnt skills.
Refresher
If there has been a change in legislation nationally or an organisation has amended its policy locally, staff will need to receive an update to reflect any changes
